Job Summary:

The Department of Orthopaedic Surgery and Rehabilitation Medicine at SUNY Downstate Health Sciences University is seeking a full-time PPS & Rehabilitation Admissions Manager (PT, OT or RN). The successful candidate will:

  • Report to the Rehabilitation Medicine Division Administrator.
  • Complete PPS management and the Inpatient Rehabilitation Facility Patient Assessment within the regulations and timeframes set forth by the Center for Medicare and Medicaid Services (CMS). Input data for admission and discharge into IRVEN software system within the timeframes set forth by CMS. 
  • Ensure that patients are educated regarding patient rights related to the submission of the data and provision of rehabilitation services. 
  • Screen potential rehabilitation patients using the Inpatient Rehabilitation Facility Patient Assessment  Instrument to determine suitability for admission. 
  • Advise physicians on the appropriateness for transfer to the service.
  • Assist in discharge planning while utilizing the case mix group as a guideline for LOS.
  • Inform all team members of target dates.
  • Assist with QAPI efforts and identify Key Performance Indicators (KPI's).
  • Attend regular interdisciplinary rehab rounds and discharge rounds for all related services. 
  • Work closely with other members of the health care team to devise and carry out specialized treatment program aimed at enabling patients to return to their normal activities.
  • Establish treatment goals and create plans of care to attain the established goals for acute rehab patients. 
  • Submit all necessary paperwork, as well as electronic medial records and daily billing according to the department policy and procedures.
  • Educate related services regarding what the rehabilitation unit can offer to hospital inpatients. 
  • Participate in departmental in-services.
  • Educate referral sources on admission criteria for the service.
  • Perform outreach to other community facilities to promote the use of our rehabilitation services.
  • Provide supervision and guidance for Rehab/Therapy Aides.
  • Perform work in a dynamic environment across multiple settings, while frequently communicating with team members as necessary and appropriate. 
  • Be a team player and a role model for other staff members and students.

Required Qualifications:

  • Master's or Doctor of Physical Therapy Degree, or a Bachelor's or Master's Degree in Occupational Therapy, or a Bachelor of Science or Master of Science in Nursing Degree.
  • New York State Physical Therapist or Occupational Therapist or Registered Nurse Licensure.
  • 3+ years of experience in an inpatient rehabilitation unit.

Preferred Qualifications:

  • Experience with IRF PAI , PPS, and IRVEN data entry software preferred. 

Work Schedule:

Monday to Friday; 8:00am to 4:00pm

Salary Grade/Rank:

SL-4

Salary Range:

Commensurate with experience and qualifications
