Job Summary:

The Department of Medicine, Hematology and Oncology at SUNY Downstate Health Sciences University is seeking a full-time Patient Navigator / TH Staff Assistant 2. The successful candidate will:

  • Participate in quality care meetings.
  • Provide patient outreach and appointment making for preventative and chronic care services, pre-visit reminders, and follow-up with no-show appointments via phone, letter, and/or secure messages via patient portal; this includes collaboration with the Managed Care Office and use of patient registries, online managed care portals or other data tools.
  • Provide tracking and follow-up on labs, imaging and specialty referrals. 
  • Assist clinicians with pre-visit planning, including checking patient information on the Regional Health Information Exchange, patient portal or other electronic systems.
  • Attend pre-session huddles.
  • Provide and/or exchange patient information with other care providers, as appropriate and/or required.
  • Connect patient to the needed clinical and non-clinical resources, such as social work, insurance assistance, medication, and transportation, as needed. 

Required Qualifications:

  • High School Diploma.
  • 3+ years of experience working with patients in a fast-paced clinical environment.

Preferred Qualifications:

  • Associate's Degree preferred. 

Work Schedule:

Monday to Friday; 9:00am to 5:00pm (Full-Time)

Salary Grade/Rank:

SL-1

Salary Range:

Commensurate with experience and qualifications
