The Department of Pharmacy at SUNY Downstate Health Sciences University is seeking a full-time Internal Medicine Senior Clinical Pharmacist.
Under the direction of the Director of Pharmacy and Associate Director of Clinical Pharmacy Services, the Internal Medicine Senior Clinical Pharmacist is responsible for the provision of clinical pharmacy services to the Adult Internal Medicine population at the University Hospital at Downstate.
The Internal Medicine Senior Clinical Pharmacist works as part of a multidisciplinary team to ensure the delivery of safe, evidence-based, cost-effective and individualized drug therapy for internal medicine patients. The Internal Medicine Senior Clinical Pharmacist provides support of centralized and de-centralized medication use systems. The Internal Medicine Senior Clinical Pharmacist also serves as a Pharmacy Department liaison and subject matter expert in the Internal Medicine (Medical/Surgical) Patient Care Areas.
Additionally, the successful candidate will:
- Participate as a vital clinical team member during interdisciplinary rounds to promote safe and cost-effective pharmaceutical care
- Monitor pharmacotherapy and recommend adjustments to medication regimens by utilizing appropriate pharmacokinetic/pharmacodynamics principles, evidence-based medicine, and patient-specific factors
- Obtain accurate medication histories as needed, confirm appropriate and updated documentation in the Electronic Health Record (EHR), and ensure accurate medication reconciliation has been performed during inpatient admissions
- Provide discharge counseling as needed to patients and/or caregivers on medication therapy in conjunction with physicians and nurses
- Serve as the primary drug information resource and pharmacist liaison for the Internal Medicine service
- Detect and report clinical interventions, medication safety events and adverse drug reactions
- Participate in the hospital’s medication safety initiatives
- Serve as a member of the Pharmacy & Therapeutics Committee, pharmacy-specific and various institutional committees
- Participate in departmental and hospital-wide initiatives for quality improvement
- Assist in the review and development of policies, procedures and guidelines to promote safe and effective medication use
- Collaborate with medical staff, nursing, other members of the healthcare team, pharmacy and hospital administration in preparations for the Joint Commission accreditation and to address any deficiencies identified
- Assist with Pharmacy Department formulary management
- Design and deliver educational programs for pharmacy staff and other healthcare professionals including medical and nursing staff to promote the safe and effective use of medication therapy
- Identify opportunities for improvement and create requests for changes in the EHR that will facilitate safer medication ordering and administration
- Responsible for the clinical training of pharmacy residents, pharmacy students, or other students completing rotations in Internal Medicine
- Participate in scholarly activities including peer-reviewed publications, medication use evaluations, clinical research projects, and cost-savings initiatives
