Job Summary:

The Department of Social Work at SUNY Downstate Health Sciences University is seeking a full-time TH Staff Assistant 2 / Patient Advocate, Mother Cabrini Health Foundation. The successful candidate will: 

  • Be responsible for screening patients.
  • Conduct social determinant of Health (SOOH) screenings for inpatients and supporting the patients

  • Help patients through the screening and follow-up of a medical/psychosocial conditions.

  • Help patients set up appointments, and provide financial, legal, and social support/referrals and communicating with healthcare providers.

  • Assist patients communicate with their healthcare providers so they can get the information they need to make decisions about their care.

  • Identify barriers to care such as logistics, transportation, or language barriers etc.

  • Gather and document patient issues, barriers, or concerns. May also need to provide reports for management review.

Required Qualifications:

  • High School Diploma.
  • Excellent communication and interpersonal skills.
  • Empathetic attitude, with an ability to offer emotional support.
  • Passion for the proper care and well-being of patients.

Preferred Qualifications:

  • Bachelor's Degree in a healthcare-related field preferred.
  • 2+ years experience in a healthcare setting preferred.
  • Knowledge of healthcare systems, insurance plans, and patient resources.

Work Schedule:

Monday to Friday; 9:00am to 5:00pm (Full-Time)

Salary Grade/Rank:

SL-1

Salary Range:

Commensurate with experience and qualifications
