Job Summary:

The Department of Social Work at SUNY Downstate Health Sciences University is seeking a full-time Community Relations Assistant, Mother Cabrini Health Foundation. The successful candidate will:

  • Report to the Social Determinants of Health (SDOH) Program Manager.

  • Advocate for community needs and resources.

  • Make referrals to community-based organizations for patient who screen positive for Social Determinants of Health (SDOH)

  • Follow up with patients as needed to ensure that needs are addressed appropriately.

  • Provide culturally appropriate health education and information

  • Assist individuals in understanding their health conditions and treatment plans

  • Facilitate communication between patients and healthcare providers.

  • Offer social support as needed. Coordinate and implement health promotion activities and programs.

  • Collect data and maintain accurate records of referrals

  • Refer individuals· to appropriate healthcare and social services

  • Follow up with all referrals for community-based organizations for outcomes

  • Organize and participate in community events to raise health awareness

  • Support individuals. in navigating the healthcare system

Required Qualifications:

  • High School Diploma or equivalent.
  • Excellent communication and interpersonal skills, empathetic attitude, with an ability to offer emotional support.

  • Passion for the proper care and well-being of patients.

Preferred Qualifications:

  • Bachelor's Degree in a healthcare or related field is preferred.
  • 2+ years of experience in a healthcare setting preferred.

  • Knowledge of healthcare systems, insurance plans, and patient resources, and an understanding of medical terminology preferred. 

Work Schedule:

Monday to Friday; 9:00am to 5:00pm (Full-Time)

Salary Grade/Rank:

SL-1

Salary Range:

Commensurate with experience and qualifications
