Job Summary:

The Department of Transplant Surgery at SUNY Downstate Health Sciences University is seeking a full-time Assistant Director of Nursing / Nurse Practitioner. The successful candidate will:

  • Manage the coordination of care for patients who are in post-transplantation phase of care. 
  • Attend post-transplant clinic and perform independent clinical assessments of transplant recipients.
  • Review laboratory, radiographic, and diagnostic test results; monitor for adverse effects and changes which could affect the patient. 
  • Collaborate with attending surgeons and nephrologists related to results and plan of care.
  • Maintain continuity of care between inpatient and outpatient services. 
  • Provide patient education related to post-transplant care.
  • Develop and implement protocols for health maintenance, screening, and quality of life management after organ transplantation.
  • Create referrals to sub-specialty care, as needed.
  • Participate in multi-disciplinary care coordination.
  • Participate and assist with quality improvement initiatives and programs.
  • Perform any additional duties, as required and needed by the department. 

Required Qualifications:

  • Master's of Nursing Degree.
  • New York State Registered Nurse and Adult/Family Nurse Practitioner Licensure. 
  • Current national certification as an Adult/Family Nurse Practitioner.
  • 10+ years of nursing experience. 

Preferred Qualifications:

  • 3+ years of experience as an outpatient setting Nurse Practitioner.
  • Experience in the care of chronically ill patients, patients with suppressed immune status, and patients with socio-medical issues which can affect the success of kidney transplant. 

Work Schedule:

Monday to Friday; 9:00am to 5:00pm

Salary Grade/Rank:

SL-5

Salary Range:

Commensurate with experience and qualifications
